Transitional Care Management

Transitional Care Management, or TCM, helps eligible patients move from a hospital or other qualifying facility back into the community. It connects discharge information with primary care through communication, medication review, and coordination of the next steps.

A clinician and patient discussing an individualized care plan.

Your care, in context.Individual assessment. Clear next steps. Connected follow-up.

What your care may include.

The scope of transitional care management is guided by your clinical needs and the information reviewed at your visit. The following areas can form part of the conversation or care plan when appropriate.

  • Timely post-discharge communication
  • Discharge-instruction and medication review
  • Primary-care and specialist coordination
  • Pending-test and community-resource review

Important expectations

Program eligibility and timing must be assessed. If symptoms worsen or an emergency develops, seek emergency care rather than waiting for follow-up.

A clearer start to your care.

Contact the office promptly after discharge. Bring discharge paperwork, your updated medication list, and details of home-health or specialist plans.

When arranging care, ask the office about availability, appointment type, required records, costs, and any insurance questions. Do not change medication or begin special test preparation unless your clinician instructs you to do so.
