Remote Patient Monitoring

Remote Patient Monitoring, or RPM, allows appropriate health measurements to be collected outside the clinic and reviewed by the healthcare team. When suitable, these measurements can support discussion of trends and timely adjustments to an individualized care plan.

What your care may include.

The scope of remote patient monitoring is guided by your clinical needs and the information reviewed at your visit. The following areas can form part of the conversation or care plan when appropriate.

  • Clinically appropriate home measurements
  • Blood pressure, heart rate, weight, or other selected data
  • Care-team review of trends
  • Follow-up and care-plan coordination

Important expectations

RPM is not continuous emergency surveillance. Eligibility, equipment, review schedules, costs, and response instructions must be confirmed with the care team.

A clearer start to your care.

When arranging care, ask the office about availability, appointment type, required records, costs, and any insurance questions. Do not change medication or begin special test preparation unless your clinician instructs you to do so.
