Chronic Care Management

Chronic Care Management, or CCM, provides ongoing support for eligible patients living with chronic conditions that require continued medical management. The program extends coordination beyond individual appointments and helps connect the different parts of an individualized care plan.

What your care may include.

The scope of chronic care management is guided by your clinical needs and the information reviewed at your visit. The following areas can form part of the conversation or care plan when appropriate.

  • Individualized comprehensive care planning
  • Medication and chronic-condition review
  • Specialist and community-resource coordination
  • Preventive-care tracking and ongoing communication

Important expectations

Eligibility, participation, consent, and potential costs are reviewed before enrollment. CCM does not replace office visits or emergency care.

A clearer start to your care.

Ask the office whether CCM may fit your needs. Bring your condition and medication lists and information about your care team.

When arranging care, ask the office about availability, appointment type, required records, costs, and any insurance questions. Do not change medication or begin special test preparation unless your clinician instructs you to do so.
