Insurance & Financial Assistance

We want cost and coverage questions to be clear before your visit. Allegiant Community Care can help you review your plan information and discuss available payment options. Coverage, network status, and patient responsibility depend on your specific plan and services.

Payment & Assistance Options

Medicaid and Medicare: Bring your current insurance card and a photo ID. If you have Medicaid, bring your Medicaid ID or mihealth card when applicable. Medicare coverage and patient responsibility vary by service and plan.

Financial assistance questions: Ask our office whether an income- and family-size-based payment option is available for your circumstances. Availability and eligibility must be discussed with the office; assistance is not automatic or guaranteed.

Private plan verification: We can help check the plan information you provide, but verification is not a promise of payment. Before care, use your insurer’s plan-verification directory and confirm that Allegiant Community Care and the planned service are covered. Deductibles, co-insurance, copays, referrals, prior authorization, and out-of-network costs may apply.

  • Have your member ID card ready.
  • Ask your insurer about network status, benefits, deductibles, co-insurance, copays, referrals, and prior authorization.
  • Call 517-541-1000 to discuss coverage or payment questions before your appointment.

Our Commitment to Affordability

As a nonprofit community practice, Allegiant Community Care is committed to making care conversations respectful and understandable. Our team can explain what we know, identify questions for your health plan, and discuss payment options that may be available. Please call before your visit so we can review your situation; no website can determine your final benefit or balance.
